A brochure, price information and an application for enrollment are available at the
Children's Village office.
A completed application may be mailed or delivered to the Children's Village office. A non-refundable registration will be collected with the application.

- Child's Preadmission Health Evaluation
- Immunization record
- Child's Preadmission Health History
- Signed & Dated "Notice - Parent's Rights" Receipt
- Student Enrollment Record & Consent for
- Medical Treatment
- Personal Rights - Day Care Facilities
- Sunscreen Permission
