Admissions

A brochure, price information and an application for enrollment are available at the
Children's Village office.

A completed application may be mailed or delivered to the Children's Village office. A non-refundable registration will be collected with the application.

The following items must be received before the child's first day

  • Identification & Emergency Form
  • Parent Handbook & Admission Agreement
  • Child's Preadmission Health Evaluation
  • Immunization record
  • Child's Preadmission Health History
  • Signed & Dated "Notice - Parent's Rights" Receipt
  • Student Enrollment Record & Consent for
  • Medical Treatment
  • Personal Rights - Day Care Facilities
  • Sunscreen Permission
